Pat Fontana

Pat Fontana holds a B.A. in Mathematics and a B.A. in Education from Syracuse University, an M.A. in Theological Studies from Holy Apostles College, and a Health IT Certification from Cornell University.

Pat’s previous roles at FDRHPO include Health IT Specialist, Rural Health Network Manager, and most recently, Director of Population Health. He also has experience as an instructor in various disciplines, including mathematics, computer programming, and health IT. As FDRHPO Deputy Director, Pat oversees FDRHPO’s program areas that support our regional stakeholders’ efforts toward the improvement of population health, behavioral health, and healthcare workforce.

Pat is a native of the North Country and currently resides in Watertown with his wife and two sons.
