Continuity of Patient Care - Patient Care Documentation

There are three methods of delivering prehospital care reports electronically to destination facilities within New York state.

 

  1. Hospital Hub ~ the REMSCOs listed below use a complimentary software program where all ePCRs sent to the State Elite Site and the Regional Elite Sites are made available to the destination hospital immediately upon receipt by the Regional Elite site

  2. Elite Viewer ~ the remainder of the state outside New York city use a complimentary software program where all PCRs sent to the State Elite Site not visible on Hospital Hub or Exchange ER (New York City) are available to destination hospital immediately upon receipt by the State Elite site

  3. ExchangeER ~ a service contracted by the 911 system in New York City for those EMS agencies using HealthEMS (the Sansio product) that provides the ePCR to destination hospitals, the remainder of ePCRs not documented in HealthEMS may be located on Elite Viewer

 

When documenting patient care, all EMS agencies should document the name and code of the destination facility whenever known.  The documentation included on the PCR/ePCR provides vital information, which is necessary for continued care at the hospital.  As part of transferring the patient to the Emergency Department Staff (or other receiving hospital unit’s staff), the agency must provide an appropriate medical record that includes the demographic, event/incident, assessment,  findings and treatment details upon delivery of the patient. The above-noted pathways detail how an EMS agency may ensure an ePCR is received by the destination facility.  Additionally, all EMS agencies are reminded that prior to departing the hospital the ePCR must be submitted through to the hospital electronically or a written document that includes demographic, event/incident, assessment, findings and treatment details.

 

Every EMS agency should do their utmost to ensure the ePCR is received by the destination facility. Any questions about ePCR delivery to the destination facility may be sent to EMSData@health.ny.gov.  Any hospitals not accessing Elite Viewer or Hospital Hub may contact the Data and Informatics Unit at EMSData@health.ny.gov.
