Role: Director of Population Health
Pat Fontana holds a B.A. in Mathematics and a B.A. in Education from Syracuse University, an M.A. in Theological Studies from Holy Apostles College, and a Health IT Certification from Cornell University.
His previous roles include Health IT Specialist, Rural Health Network Manager, and Instructor in various disciplines including mathematics, computer programming and health IT. As Director of Population Health, he provides support to regional stakeholders by facilitating efforts toward the improvement of overall population health, enhancement of patient satisfaction, and the reduction of per capita cost of care.
Pat is a native of the North Country and currently resides in Watertown with his wife and two sons.
